These ulcers may be painless due to neuropathy, which often delays presentation and treatment. Signs of infection include redness, warmth, swelling, purulent discharge, and systemic features such as fever in severe cases. Advanced disease may result in tissue necrosis and necessitate partial or complete amputation of the affected limb.
The impact of diabetic foot syndrome extends beyond physical health. It significantly reduces quality of life, limits mobility, and is associated with psychological distress, including anxiety and depression. From a public health perspective, DFS imposes a substantial economic burden due to prolonged hospital stays, repeated surgical procedures, long-term wound care, and rehabilitation.
Prevention is the cornerstone of managing diabetic foot syndrome. Effective glycemic control is fundamental, as sustained hyperglycemia accelerates neuropathy and vascular disease. Regular foot examination by healthcare professionals and daily self-inspection by patients are crucial for early detection of risk factors such as calluses, deformities, skin breaks, or color changes. Patient education plays a vital role, emphasizing proper foot hygiene, nail care, avoidance of walking barefoot, and prompt reporting of any foot abnormalities. Appropriate footwear that reduces pressure and prevents injury is also essential, particularly for high-risk individuals.
Management of diabetic foot syndrome requires a multidisciplinary approach involving physicians, podiatrists, nurses, vascular surgeons, and infectious disease specialists. The primary goals are to control infection, promote wound healing, restore adequate blood flow, and prevent recurrence. Wound care includes regular debridement of necrotic tissue, maintenance of a moist wound environment, and offloading pressure using specialized footwear or casts. Infected ulcers require prompt antibiotic therapy, guided by culture results whenever possible.
