Proper Preoperative Planning:
Assess the cataract density, pupil size, anterior chamber depth, and zonular status.
Choose the right phaco machine settings and phaco tip based on nucleus hardness.
Counsel the patient regarding the procedure and postoperative expectations.
Clear Corneal Incision and Capsulorhexis:
Make a self-sealing clear corneal incision (around 2.2 to 2.8 mm) at the steep axis to reduce postoperative astigmatism.
Perform a continuous curvilinear capsulorhexis (CCC) of approximately 5 to 5.5 mm for optimal nucleus manipulation and IOL centration.
Hydro dissection and Hydro delineation:
Carefully inject fluid under the anterior capsule to separate the nucleus from the cortex and posterior capsule.
Rotate the nucleus to ensure it is freely mobile within the capsular bag.
Phacoemulsification of the Nucleus:
Apply the “Divide and Conquer” or “Stop and Chop” technique depending on nucleus density.
Use sculpting to create grooves and cracks, followed by quadrant removal with adequate phaco power and vacuum while protecting the posterior capsule.
Cortical Clean-Up and Posterior Capsule Polishing:
Gently aspirate the cortical material using bimanual or coaxial irrigation/aspiration (I/A).
Polish the posterior capsule to reduce the risk of posterior capsular opacification (PCO).
IOL Insertion and Wound Closure:
Inject a foldable intraocular lens (IOL) into the capsular bag, ensuring correct centration.
Remove all viscoelastic from behind the IOL and anterior chamber.
Hydrate the wound edges to achieve a watertight seal and check for wound integrity.
